What is C.H.E.N. ?
We are Community Health Equity Navigators or simply, the C.H.E.N. Team.We are a division within Abrazar Inc, a non-profit service provider under contract with the Office on Aging since its founding in 1975.Our mission is to assist Orange County's underserved communities by helping community members locate, access and utilize resources.
Our Services:
1:1 Non-Clinical Counseling
Our Community Health Equity Navigators provide non-clinical and culturally proficient crisis counseling services to Orange County community members of all ages. C.H.E.N. team also helps community members locate and utilize resources by researching programs, learning about their eligibility requirements, and finally referring our clients, with follow-ups to ensure our clients received the support they need.
